Contestants should download and complete the necessary forms below.  Remember to submit them along with your photos, talent CD and entry fee (Teen only) to the address below.  Remember, Miss talent time limit is now 90 seconds and Teen talent time limit is still 2 minutes.  Please contact us with any questions you may have.

Each Miss contestant in the pageant will be required to raise $100 for the Children's Miracle Network.  After filling out and sending the materials above please register with CMN at:
 
 
If this is the first time you have raised donations you will need to click on "New Contestants" and set up your profile.  You must upload a .jpeg picture of yourself.  Then you can send emails to friends and family to get donations.
